This video presents a comprehensive analysis of periodontal disease as a major global health challenge affecting over one billion people worldwide, with far-reaching consequences extending beyond oral health to significant systemic and socioeconomic impacts. The narrative review synthesizes evidence from 175 selected studies demonstrating clear associations between periodontal inflammation and increased risks of cardiovascular disease, diabetes mellitus, adverse pregnancy outcomes, respiratory infections, and neurodegenerative disorders, mediated through chronic inflammatory pathways and microbial dissemination. The findings reveal substantial healthcare disparities in disease prevalence and outcomes, particularly affecting underserved populations, while economic analyses indicate considerable productivity losses and healthcare costs attributable to untreated periodontal conditions. The evidence underscores the critical need for integrated public health approaches that incorporate oral health into primary care systems and chronic disease management programs, emphasizing that effective policy interventions must address both biological mechanisms and social determinants of health to reduce the global burden of periodontal disease and its systemic consequences. The review concludes that multidisciplinary collaboration among dental professionals, physicians, public health experts, and policymakers is essential to develop cost-effective prevention strategies and implement population-wide interventions that can mitigate this preventable source of morbidity and healthcare expenditure.
